Please print and complete the  Medical Application Form, Mental Health Checklist and Sleep Apnoea Questionnaires from the links below and bring them along with some photo ID.   If you have not had an appointment with me before, please scan me a copy of your current EASA certificate prior to our meeting.

If you wear glasses or contact lenses, please bring along a copy of you most recent prescription. If you wear contact lenses, you will need to be able to remove them so I can check your uncorrected vision.
